A coroner, who oversaw the Charlotte Comer inquest previously from Berkshire, said “serious failures” by the Herefordshire and Worcestershire Health and Care NHS Trust contributed to the death of the 30-year-old woman, from Earls Croome, near Malvern.
Worcestershire Chief Coroner David Reid recorded a verdict that Ms Comer died of an overdose with her family at her bedside at Worcestershire Royal Hospital on 20 July 2021 after taking the overdose on the day former. Reid said he discovered that Miss Comer died as a result of the NHS trust’s failures and ordered a rare preventative report of future deaths to be completed, such were the concerns he had with the trust’s “serious failures”.

His investigation heard that Ms. Comer suffered from body dysmorphic disorder, in which a person spends a lot of time worrying about her appearance, and struggled with eating disorders and self-harm. Miss Comer moved from Berkshire to Worcestershire in 2018 and took an overdose in the past that put her in a coma for three weeks, before recovering.
Ella miss Comer had eight care coordinators, who are there to help people with complex needs, and there were five months in 2021 when she was without one. The inquest also heard that Miss Comer had been referred to The Priory for treatment for BDD in 2021.
But, it transpired that her referral was put on hold by a senior physician because she thought Ms. Comer did not meet the criteria for her place to be funded locally. The doctor had not understood that the place would be centrally funded through England’s NHS, so the referral should not have stopped.
Following this, family and friends said Miss Comer was “confused” and had a “loss of hope”. The coroner said the key factors were the number of care coordinators, that trust had not been focused on BDD and her loss of hope. after being told the remission had been paused had contributed to Ms. Comer’s decision to overdose.
After the verdict, Liam Dolan of the trust answered questions about what steps had been taken to address the areas where the coroner had raised concerns for him. Mr. Dolan said the lack of care coordinators at the time was due to the disease and the workload of the care coordinators who were currently treating 100 patients each.
He said care coordinators now had 20 to 25 patients each, down from the national guideline of 30, and there was better staff retention. When asked if the referral funding bug could happen now, he offered assurances that he wouldn’t. Despite this, the coroner said that he was still not satisfied and ordered the report to prevent future deaths to be completed.
